Didn't see this before, but the limit for this MRI is 3 per household and you have to mail them seperatly. It's down at the bottom of the form in really really small print.

I also didn't need to send in UPC's. Just wrote them on the paper provided and circled the receipt.

For the poster who got the rejection letter, I'd call them on that one. $5 is $5.
